Il peut aussi concerner les \u00e9l\u00e8ves intellectuellement pr\u00e9coces.<\/span><\/p>\n<p><span style=\"font-family: ABeeZee;\">Le PPRE est mis en place par le directeur de l\u2019\u00e9cole ou le chef d\u2019\u00e9tablissement, \u00e0 l\u2019initiative des \u00e9quipes p\u00e9dagogiques. Il prend la forme d\u2019un document qui permet de formaliser et de coordonner les actions con\u00e7ues pour r\u00e9pondre aux difficult\u00e9s que rencontre l\u2019\u00e9l\u00e8ve.<\/span><\/p>\n<p><span style=\"font-family: ABeeZee;\">Il n\u2019existe pas de mod\u00e8le unique de PPRE. Ces adaptations sont regroup\u00e9es dans un livret.\u00a0<strong><br \/>\n<\/strong>Ces adaptations peuvent \u00eatre compl\u00e9t\u00e9es par l'\u00e9quipe, et tous les partenaires. Ce n'est pas un \"catalogue\" de supermarch\u00e9, mais bel et bien une source d'informations, de communications connue de tous les partenaires. Ce dossier comportera aussi\u00a0:<\/p>\n<ul>\n<li>Le\u00a0 \u00ab\u00a0 Formulaire de demande \u00e0 la MDPH\u00a0\u00bb<\/li>\n<li>Un certificat m\u00e9dical de moins de 3 mois renseign\u00e9 par le m\u00e9decin sp\u00e9cialiste qui suit l\u2019enfant<\/li>\n<li>Un bilan \u00e9tabli par le psychologue<\/li>\n<li>Si possible, une copie du Livret d\u2019Evaluation ou des derniers bulletins trimestriels<\/li>\n<li>Tout autre bilan effectu\u00e9 par les diff\u00e9rents sp\u00e9cialistes param\u00e9dicaux qui suivent l\u2019enfant<\/li>\n<\/ul>\n<hr \/>\n<p><em>\"Le projet personnalis\u00e9 de scolarisation est mis en \u0153uvre par le ou les enseignants de l'\u00e9l\u00e8ve.
